You will not receive a membership card. Your plan’s effective date will be on file with our office.

Plan Includes (per patient, per year)

  • 3 Exams total. In addition to your 2 six month check-up exams, you can have one other exam. (Example: You have a toothache and you need an emergency exam for the dentist to check it.)

  • 2 Cleanings

  • 1 Panoramic x-ray

  • 1 Set of bite-wing x-rays plus 2 periapical x-rays (PA’s)

  • 1 Diagnostic x-ray (Example: When you come in for your toothache exam, the dentist takes an x-ray of that tooth to help with your diagnosis)

  • 15% Discount on Other Comprehensive Services
